As we get into the summer term, I would like to remind all families to bring back the slip on the transition form to start building links with your child's chosen primary school. The permission slip ensures your child's key worker can arrange to meet your new teacher and share the achievements your child has achieved. 

 

If you would like to meet your child's key worker to discuss progress and any thoughts you have about the upcoming transition to school, please plan a meeting with your child's key worker.
